Urgent Medical Care and Doctor Availability

​

Your health is our priority. If you're experiencing an urgent medical concern, please don't wait for our clinic to open. Instead, we strongly advise you to go to your nearest hospital for immediate care. This is in line with Alberta Health guidelines, and our clinic's hours are Tuesday, Wednesday, and Thursday.

​

Please understand that our doctor is fully dedicated to patient care during clinic hours and is unable to make phone calls. We also appreciate your understanding that our cancellation list is currently very long, and we're working diligently to accommodate all patients.

​

Zero Tolerance Policy

 

At Shape MD Medical Clinic, we are committed to providing a safe and respectful environment for everyone. We have a strict zero-tolerance policy for any form of abuse directed towards our staff or doctors. Verbal abuse, threats, or any disruptive behaviour will not be tolerated. We reserve the right to refuse service to any patient who violates this policy, ensuring the well-being of our team and the integrity of our clinic.

 

Form Fee Policy

 

Please be advised that effective June 1, 2025, a $100 fee will be applied to a patient's initial medical form request.

All subsequent forms submitted in future follow-ups will incur a $50 fee.

This policy is being implemented due to the significant time and effort required by our physicians to thoroughly review patient medical histories for accurate form completion. Furthermore, in the event of delays in form processing by a patient's insurance provider, the responsibility for upfront payment of cosmetic or medical injection services will lie with the patient. At this time, no exceptions to this policy can be accommodated.

 

No Show Policy

At Shape MD Medical Clinic, we are dedicated to providing timely and accessible care to all our patients. Due to an extensive waitlist and a commitment to accommodating those who are dedicated to their health journey, we are implementing a revised no-show policy, effective January 1, 2025. ​

 

Any patient who fails to attend a scheduled appointment without prior notification will be permanently discharged from Shape MD Medical Clinic, with no exceptions. A $100 fee for each no-show is required to reinstate an appointment. ​

 

We understand that unforeseen circumstances can arise, and we encourage patients to notify us as soon as possible if they need to cancel or reschedule an appointment. This allows us to offer that valuable time slot to another patient in need. We appreciate your understanding and cooperation as we strive to best serve our community.

Discharge Policy

 

​To manage our patient roster efficiently and ensure we can best serve those actively engaged in their health journey, we will permanently close all inactive patient files where the patient has not seen a doctor at our clinic for a period of one year or more.

​

Should you wish to resume care with Shape MD Medical Clinic after your file has been closed, you will be required to undergo an initial consultation with one of our medical assistants to reactivate your file. Please note that same-day booking with a doctor will not be permitted for reactivated files, and appointments will be scheduled based on availability after the initial consultation with our medical assistant. We encourage you to maintain regular appointments to ensure continuity of care and avoid file closure.

Prescription Refill Policy

 

Effective March 1, 2025, please note important changes regarding prescription refills. Given the high patient volume and demanding physician schedules, our medical clinic will no longer call in prescription refills to pharmacies. Instead, we now require your pharmacy to submit a fax request for all prescription refills to our office at 780-988-2609. This procedural change will allow our doctor to dedicate their full attention to patient care during appointments.

 

Furthermore, please be aware that if a patient has not seen the doctor within the past year, a prescription refill will not be granted by fax, and an in person follow-up appointment will be required. If a fax request cannot be arranged, patients will also need to book a regular appointment to obtain a new prescription or refill. We appreciate your understanding and cooperation as we implement these more efficient refill processes.

Botox / Filler Touch Up Policy

 

To ensure optimal results and address any minor adjustments needed, Botox / Filler touch-up appointments should be scheduled within one to two weeks following your initial injection. Please note that any Botox / Filler appointments requested after this two-week window will be considered a new treatment and will incur an additional out-of-pocket expense. We encourage you to book your touch-up promptly if you feel it's necessary.

Benign Skin Lesion Removal Policy

 

Please be advised that the removal of benign skin lesions such as seborrheic keratosis, milia, sebaceous hyperplasia, syringomas, and skin tags are considered cosmetic procedures and are not covered by Alberta Health Care. These procedures are private pay, and patients interested in their removal must first book a complimentary skin consultation. This consultation will allow our medical professionals to examine the extent of the lesions and discuss the associated private fees before proceeding with any treatment.

New Consultation Policy

 

Effective March 1, 2025, all new and never-before-seen patients are required to undergo an initial complimentary skin consultation. Please note that direct bookings with Dr. Jennifer Tse are not available, and we do not offer same-day appointments with her. This mandatory consultation allows the necessary time to thoroughly review your medical history and ensure there are no contraindications before scheduling an appointment with Dr. Jennifer Tse, ultimately enabling us to understand your specific needs and make the most appropriate recommendations for your skin health journey

Language Barrier Policy

​

As per clinic policy, all new patients who do not speak English must be accompanied by a translator to ensure proper understanding and communication regarding medical issues. This policy is in place to protect the patient and ensure they fully comprehend all aspects of their medical care, including diagnoses, treatment plans, and potential risks.

 

If you arrive for your appointment without a translator, you will be required to rebook. Please note that our clinic does not accept translating apps due to the time-consuming nature of these tools, which can disrupt the flow of appointments and compromise the quality of your medical consultation. Having a human translator present allows for a thorough discussion of your health concerns, ensuring that you can make informed decisions about your medical care and that our healthcare providers can proceed with treatment safely and effectively.

No Friends/Family Discounts

 

Please be advised that effective March 1, 2025, Shape MD Medical Clinic will no longer be offering friends and family discounts for cosmetic procedures. This adjustment is a direct response to the increasing pressures of inflation and economic tariffs, which have significantly impacted our operational costs. Despite these rising expenses, we've maintained our pricing structure for the past seven years. Going forward, all patients will be charged the regular price for all cosmetic procedures. We appreciate your understanding and continued support.
